Introduction: Sialolithiasis the most common pathology of the main salivary glands, and may be responsible for an obstructive syndrome characterized by the occurrence of glandular infection syndrome with swelling that can be confused with an odontogenic cellulitis infectious. Observation: We report the case of a 22-year-old patient, who was referred to us for the management of low suppurative cellulitis related to wisdom tooth development accident on the right side (48 impacted) and at the same time, he presented with an ipsilateral glandular obstructive syndrome related to sialolithiasis in the posterior portion of the Wharton duct. The extraction of the causal tooth and the excision of the stone by trans oral approach allowed the disappearance of the infectious process and the preservation of the gland thanks to a conservative technique of excision of the obstacle, despite its posterior location. The aim, of this article is to highlight the importance of the anamnesis, the clinical examination and the good interpretation of the radiographic assessment. Moreover, to raise the difficulty of the therapeutic choice of lithiasis of the submandibular gland which results from their posterior location.
