SÚHRN Izovalerová acidúria (IVA, OMIM # 243500) je autozómovo-recesívna porucha s hromadením metabolitov izovaleryl-CoA: izovalerylkarnitínu (C5), 3-OH-izovalerátu a izovalerylglycínu (IVGL). IVA je súčasťou rozšíreného novorodeneckého skríningu (RNS) dedičných metabolických porúch (DMP) tandemovou hmotnostnou spektrometriou (MS/MS) v suchej kvapke krvi (SKK). Primárnym markerom je zvýšená koncentrácia C5, ktorý reprezentuje viacero izomérov (izovalerylkarnitín, 2-metylbutyrylkarnitín, pivaloylkarnitín) a môže súvisieť aj s inou DMP alebo podávaním liečiv, napr. antibiotík s kyselinou pivalovou, amoxicilínom, kozmetických prípravkov s obsahom neopentanoátu. V práci prinášame výsledky následného testovania po pozitívnej identifikácii pre IVA u novorodencov zo západoslovenského regiónu. ABSTRACT Isovaleric aciduria (IVA, OMIM# 243500) is an autosomal-recessive disorder resulting in the accumulation of isovaleryl-CoA metabolites: isovalerylcarnitine (C5), 3-OH-isovalerate, and isovalerylglycine (IVGL). IVA is included in the expanded newborn screening (NBS) in Slovakia for inherited metabolic disorders (IMD) by tandem mass spectrometry (MS/MS) in a dried blood spot (DBS). The primary marker of the disease is the elevated concentration of C5, however it represents several isomers (isovalerylcarnitine, 2-methylbutyrylcarnitine, pivaloylcarnitine) and may be related to other IMD or the administration of certain drugs, e.g. antibiotics containing pivalic acid and amoxicillin, or the use of cosmetic products containing neopentanoate. We report the results of the follow-up testing of infants identified for IVA by NBS from the Western Slovakia region.
