Pharmaceutical care program for patients with chronic kidney disease in the community pharmacy

Abstract

Pharmaceutical care program for patients with chronic kidney disease in the community pharmacy Patients with low glomerular filtration rate (<60ml/min/1.73m2) need to use lower doses of certain drugs and should avoid nephrotoxic drugs. The aim is to detect patients with low glomerular filtration rate, study their medication and communicate to the physician any possible need related to their medication that we detect. The study was conducted in 4 community pharmacies in Spain. Description of the Data and file structure The data are organised in two tables. One shows all patients in the study with personal characteristics and glomerular filtration rate. There are 200 but two of them were excluded. The fields used are: Excluded, Id-Patient, Date, Age, Sex, Creatinine provided by patient (the patient informs us of the result of an analysis less than three months old) Creatinine obtained by pharmacy (when the patient does not provide us with the creatinine we measure it in the pharmacy at the time), eGF, Diagnosed (the patient indicates that he/she has been diagnosed with CKD), Physician reported creatinine (creatinine provided by the doctor when he/she does a confirmation analysis). Another table lists all medications used by all patients. It is indicated whether any intervention was needed by the pharmacist and the final decision of the physician. The fields used are: Id-Patient, Id med, activ ingredient, ATC classification, prescribed dose, requested action, action carried out. Both tables are related by the Id-Patient field.
