Hiperparatiroidismo secundario en pacientes dializados del Hospital Regional de Encarnación

Abstract

La enfermedad renal crónica da lugar a alteraciones en las concentraciones de fósforo, calcio, hormona paratiroidea (PTH) y fosfatasa alcalina (FASA). Esto desencadena una patología denominada hiperparatiroidismo secundario en los pacientes que la padecen. Esta enfermedad contribuye al alto riesgo de fracturas y mortalidad cardiovascular. Nos propusimos determinar la frecuencia y distribución según sexo y edad de hiperparatiroidismo secundario en los pacientes dializados. Además, nos propusimos conocer los valores de las concentraciones de fósforo, calcio, FASA y PTH en los pacientes que presentaron hiperparatiroidismo secundario y analizar la relación entre PTH, fósforo, calcio y FASA de los pacientes dializados. Se estudió a 33 pacientes de ambos sexos en diálisis del Hospital Regional de Encarnación, Paraguay. El promedio de edad fue de 53 ± 15 años. El 48% (n=16) correspondió a mujeres. Se encontró que el 52% (n=17) tenía hiperparatiroidismo secundario. La mayor prevalencia estaba en mujeres con edades de 51 a 70 años (47%). El 66% de los pacientes presentaron concentraciones de fósforo elevados (> 5,5 mg/dl). El 76% presentó valores de calcio dentro de los rangos considerados adecuados. El 67% tenía valores elevados de FASA (>150U/l). Por otra parte, se encontró una correlación significativa (𝑃 = 0.006) de PTH con las concentraciones de FASA. La presencia de hiperparatiroidismo secundario fue elevada en pacientes con enfermedad renal crónica sometidos a diálisis. Esto podría llevar a un importante deterioro de la calidad de vida e incremento de la morbimortalidad de los mismos. La FASA podría utilizarse como alternativa para el seguimiento de la mencionada patología.
