The case describes a situation of cannot ventilate, cannot intubate situation immediately after anaesthesia reversal in a 4-years old patient who underwent laryngotracheal examination under general anaesthesia. Although the case was successfully managed by doing an emergency low-tracheostomy, the real problem raised with the parents refusing to accept the life-saving tracheostomy as per their socio-cultural beliefs.
