FACTEURS PREDICTIFS DE SURVENUE DINSUFFISANCE RENALE CHEZ LES PATIENTS LITHIASIQUES : A PROPOS DE 520 CAS

Abstract

La lithiase urinaire, est une affection frequente qui touche une population jeune et se caracterise par sa recidive frequente. Lobjectifde cette etude est didentifier les facteurs susceptibles de favoriser la survenue delinsuffisance renale chez les patients lithiasiques. Dans laserie de 520 patients porteurs de lithiase urinaire pris en charge au service durologie du centrehospitalier universitaire Mohammed VI de Marrakech durant la periode allant de Janvier 2010 jusqu�� Decembre 2019, une insuffisance renaledorigine lithiasique a ete retrouvee chez 120 patients (groupe 1) soit une prevalence globale de23%, tandis que 400 patients avaient une fonction renale normale (groupe 2). A travers lanalysecomparative des param����tres cliniques, biologiques, radiologiques et therapeutiques des deuxgroupes, on a constate une difference statistiquement significative entre les deux concernant:l����ge (l����ge moyen dans le groupe 1 etait de 52 ans alors quil etait de 44 ans dans le groupe 2),le sexe (predominance masculine avec 66% au groupe 1 et 52% au groupe 2), les antecedentsurologiquesdelithiaseurinaire(45%despatientsdugroupe1contre30%despatientsdugroupe 2) et la presence de rein unique (19% et 6% respectivement dans les groupes 1 et 2). Lescalculs les plus incrimines etaient les calculs coralliformes, volumineux, de topographie renale etde si����ge bilateral. La prise en charge de la lithiase urinaire compliquee dinsuffisance renalecomporte: lantibiotherapie en cas dinfection urinaire, le drainage des voies excretrices, letraitementducalculenparticulierparlesproceduresendourologiques(nephrolithotomiepercutanee et ureteroscopie) et dans certains cas la dialyse. Les mesures preventives passent parlediagnosticprecoceetletraitementadaptedelalithiaseurinaire,lanalysemorpho-constitutionnelle du calcul et la promotion des nouvelles techniques therapeutiques au sain desservicesdurologie.
