Abstract

La fracture de la verge est une urgence urologique rare, elle survient essentiellement chez le sujet jeune. Letiologie la plus frequemment rapporte au Moyen-Orient et au pays de lAfrique du nord est la manipulation forcee de la verge en erection. La prise en charge doit etre en urgence pour prevenir les complications qui peuvent compromettre la fonction sexuelle ulterieure. Le but de notre travail est de determiner les facteurs pronostiques influençant la fonction sexuelle postoperatoire. Nous avons mene une etude analytique sur 62 patients operes pour fracture des corps caverneux dans notre service entre Janvier 2015 et Janvier 2021.La fonction sexuelle a ete evaluee par trois parametres 6 mois apres lintervention : La dysfonction erectile (DE), la presence de douleur lors des rapports sexuels et lincurvation de la verge. En postoperatoire, le taux de DE (IEEF5<20) etait de 16,1%, dincurvation de la verge de 12,9 % et seulement 9,6 % des patients ont rapporte la notion de rapports sexuels douloureux. A lanalyse multivariee, le DE etait liee au delai de consultation (p =0,04) et au siege proximal de la fracture (p =0,02). Lincurvation de la verge etait liee aussi au delai de consultation (p =0,056), au siege proximal de la fracture (p =0,033) et a lincision elective (p =0,03). Par contre aucun facteur netait lie aux rapports sexuels douloureux. Notre etude a demontre quune prise en charge precoce ameliore la fonction sexuelle postoperatoire, qui depend aussi du siege de la fracture et de la technique operatoire.
