Abstract

El divertículo de Meckel es la malformación congénita del intestino delgado debido al cierre incompleto del conducto onfalomesentérico durante el desarrollo fetal. Su incidencia es del 2 % aproximadamente. Por lo general, es asintomático, pero puede presentarse con manifestaciones clínicas de diverticulitis, hemorragia digestiva baja, oclusión o perforación intestinal. Objetivo: Exponer las características clínicas y el manejo quirúrgico actualizado en el Divertículo de Meckel. Método: Se efectuó una revisión bibliográfica a partir de la búsqueda de información en las bases de datos médicas PubMed, Scielo, ELSEVIER, Clinical Key y JAMA Surgery, con las palabras claves Divertículo de Meckel, dolor abdominal, sangrado intestinal, obstrucción intestinal. Desarrollo: El Divertículo de Meckel se localiza en el borde antimesentérico del intestino delgado, frecuentemente en los últimos 90 centímetros del íleon terminal. Es necesario un elevado índice de sospecha clínica para realizar su diagnóstico. El tratamiento es quirúrgico, mediante cirugía convencional o videolaparoscópica. La técnica quirúrgica a emplear es controversial. Está descrita la diverticulectomía; sin embargo, actualmente se aboga por la resección intestinal a 2 cm a ambos lados de su base para eliminar completamente el tejido heterotópico gástrico o pancreático y anastomosis intestinal término-terminal. Si se trata de un divertículo incidental, algunos cirujanos practican la resección; otros no la hacen para no añadir morbilidad a los pacientes. Conclusiones: El divertículo de Meckel, aunque resulta poco frecuente, puede sobreañadir morbilidad a los pacientes. De modo que, sospechar a tiempo esta entidad para que pueda diagnosticarse evitará complicaciones.
