Posible vínculo entre las miocarditis secundarias a vacunas COVID-19 y el excipiente polietilenglicol

Abstract

Nota (2026): la hipótesis de hipersensibilidad mediada por anticuerpos anti-PEG, propuesta originalmente aquí como mecanismo de la miocarditis post-vacunación, ha sido revisada. El análisis de pharmacovigilance posterior (The Fusogenic Architecture Hypothesis, DOI 10.5281/zenodo.20649943) atribuye el fenómeno a daño directo del cardiomiocito por ruptura endosomal de la LNP ionizable — un mecanismo dosis-independiente en su perfil temporal, consistente con la latencia corta y uniforme observada tanto tras la primera como tras dosis sucesivas. Se exploró la posibilidad de que el anti-PEG contribuyera selectivamente a casos de inicio retardado tras dosis ≥2, pero la evidencia disponible no sostiene esa distinción de latencia por dosis, por lo que se ha descartado como mecanismo contribuyente. Este registro no representa la posición actual sobre el mecanismo predominante de la miocarditis post-vacunal.
