Abstract

Objetivo: Revisar la información publicada sobre la dermatoporosis, haciendo hincapié en su identificación, prevención y tratamiento. Metodología: Revisión bibliográfica abarcando la década 2010-2019. Uso de diferentes recursos bibliográficos (PubMed, Cochrane Library Plus, Cuiden, IBECS y Dialnet). Combinación de descriptores DeCS, MeSH y texto libre con operadores booleanos OR y AND. Selección de artículos según criterios de inclusión y su relación con el objetivo de estudio. Lectura en profundidad de los artículos seleccionados y exposición narrativa de los resultados. Resultados: De 47 artículos recuperados fueron finalmente revisados 14. La prevalencia de la dermatoporosis es variable entre los países europeos, según investigaciones previas la tasa está entre el 21 y 38%. Existe discordancia en donde se localiza mayoritariamente la prevalencia (miembros inferiores versus superiores). Los signos clínicos más frecuentes fueron la púrpura senil y las pseudocicatrices. La principal recomendación se basa en la reducción de la exposición a factores extrínsecos, como la radiación ultravioleta, la contaminación, el tabaco y fármacos como los corticosteroides tópicos y sistémicos. Además, se debe asegurar una ingesta suficiente de proteínas, vitamina C y un aporte de líquidos adecuado. Para tratarla se propone la hidratación de la piel y la administración tópica de retinoides, ácido hialurónico, vitaminas C y E, y emolientes. Conclusiones: Se necita contar con un algún protocoloespecífico para tratar cada fase de la dermatoporosis, así como, seguimiento por la unidad de dermatología u consultas especializadas en curación de heridas. También se observó la existencia de un gran desconocimiento entre los profesionales sanitarios sobre la enfermedad (entre un 35,1% y 96,5%), siendo una posible barrera en la prevención primaria y la detección precoz, de ahí que se necesite más formación al respecto. [PREVENTION AND TREATMENT OF DERMATOPOROSIS. LITERATURE REVIEW] ABSTRACT: Purpose: To review published information on dermatoporosis, with emphasis on its identification, prevention and treatment. Methodology: Literature review covering the decade 2010-2019. Use of different bibliographic resources (PubMed, Cochrane Library Plus, Cuiden, IBECS and Dialnet). Combination of DeCS, MeSH and free text descriptors with Boolean operators OR and AND. Selection of articles according to inclusion criteria and their relation to the study objective. In-depth reading of the selected articles and narrative presentation of the results. Results: Of 47 articles retrieved, 14 were finally reviewed. The prevalence of dermatoporosis is variable among European countries, according to previous research the rate is between 21 and 38%. There is discordance in where the prevalence is mostly located (lower versus upper limbs). The most frequent clinical signs were senile purpura and pseudoscarring. The main recommendation is based on reducing exposure to extrinsic factors such as ultraviolet radiation, pollution, smoking and drugs such as topical and systemic corticosteroids. In addition, sufficient intake of protein, vitamin C and adequate fluid intake should be ensured. To treat it, skin hydration and topical administration of retinoids, hyaluronic acid, vitamins C and E, and emolients are proposed. Conclusions: There is a need for a specific protocol to treat each phase of dermatoporosis, as well as follow-up by the dermatology unit or specialized wound healing consultations. It was also observed that there is a great lack of knowledge among health professionals about the disease (between 35.1% and 96.5%), being a possible barrier to primary prevention and early detection, hence the need for more training in this regard.
