Infección por bacterias multirresistentes en pacientes con trauma cráneo encefálico del servicio de terapia intensiva del hospital Luis Vernaza, Ecuador

Abstract

Objetivo: Analizar el comportamiento de las infecciones no-socomiales por organismos multirresistentes en pacientes con traumatismo craneoencefálico del servicio de terapia intensiva del Hospital Luis Vernaza. Materiales y métodos: Se trató de un estudio observacional, retrospectivo y descriptivo en pacientes hospitalizados con diagnóstico de traumatismo craneoencefálico en el servicio de terapia intensiva del mencionado hospital, desde enero a diciembre de 2016, se emplearon los datos clínicos de las historias que se encuentran en el departamento de registros médicos. Resultados: De los 81 pacientes evaluados, el promedio de edad fue 32±14 años, el 81% (n=66) fueron hombres, el mismo porcentaje (81%) presentó algún tipo de infección no-socomial, siendo la secreción bronquial el sitio más frecuentemente aislado (38%; n=31), mientras que el 12% (n=10) fa- lleció. Entre los sujetos con infección, el 41% (n=27) presentó bacterias multirresistentes, siendo la Pseudomona aeruginosa (30%; n=8) y la Klebsiella pneumoniae KPC (26%; n=7) las bacterias más aisladas; en lo que respecta a la antibioticoterapia, el Meropenem+Vancomicina (33,3%; n=9) y Mero penem+Vancomicina+Piperazilina Tazobactam (22,2%; n=6) fueron los fármacos más empleados. Conclusión: Existe una alta frecuencia de infecciones no-socomiales por bacterias multirresistentes en pacientes con traumatismo craneoencefálico, un factor de suma importancia en la evolución de este grupo de pacientes y en el abordaje de una grave problemática en la salud pública.
