Curación de heridas en pacientes con diabetes mellitus: perspectiva clínica y molecular

Abstract

Resumen La diabetes mellitus (DM) constituye uno de los principales retos en la práctica clínica contemporánea debido a su elevada prevalencia y el amplio espectro de complicaciones crónicas que representan una proporción importante de la carga de morbilidad asociada a esta enfermedad. Entre éstas, las complicaciones asociadas a las alteraciones en la curación de las heridas son especialmente preocupantes, en particular cuando ocurren en los miembros inferiores, que se vinculan con mayor mortalidad que todas las complicaciones macrovasculares de la DM. Por lo tanto, la atención a estas heridas suele ser una de las prioridades en el seguimiento de los pacientes con DM. Además, en los pacientes con DM, los trastornos de la curación de las heridas se asocian a dolor, discapacidad, pérdida de la movilidad, depresión, estrés, ansiedad, aislamiento social y hospitalizaciones prolongadas; deteriorando de manera severa su calidad de vida. Existen numerosos problemas a abordar en el estudio de la curación de heridas en los pacientes con DM. La escasez marcada de evidencia robusta y los diseños de investigación subóptimos en la literatura disponible en la actualidad constituyen puntos a abordar urgentemente. Esta situación se ha agudizado ante el auge de nuevas alternativas terapéuticas en este escenario. En consideración de estas implicaciones, en esta revisión se discuten aspectos esenciales del abordaje clínico y el sustrato fisiopatológico de la curación de heridas en los pacientes con DM
