Metformina: más allá de la diabetes mellitus

Abstract

En los últimos 60 años, la metformina ha sido un bastión frmemente establecido en el tratamiento de la diabetes mellitus (DM), con la gran mayoría de los pacientes con esta patología consumiendo este fármaco. La metformina es prominente por sus efectos anti-hiperglicemiantes y sensibiliza- dores a la insulina, actuando fundamentalmente a través de la inhibición de la cadena respiratoria mitocondrial y la activación de la vía de la AMPK. Aunque estos son los mecanismos de acción más destacados de la metformina, se han descrito numerosos otros efectos con impactos signifcativos no sólo en la DM, sino también en varias enfermedades, como el cáncer y enfermedades neurodegenerativas, entre otras. En esta revisión se describen los hallazgos relacionados con el uso de metformina en estas patologías y sus mecanismos de acción relacionados. Los afectos antineoplásicos de la metformina están vinculados con la activación de la AMPK, junto con la inhibición de mTORC1, las Rag GTPasas, el HIF-1 y la UPR, alterando el metabolismo energético de las células, lo cual desfavorece su supervivencia y promueve la apoptosis en las células tumorales. Por otro lado, la metformina podría tener un potencial efecto de neuroprotección y de potencia- miento cognitivo. Los mecanismos propuestos se enfocan en cambios en el metabolismo de la proteína tau y en la producción de proteína 13 amiloide, además de modulación de la neuroinfamación y el estrés oxidativo en el microambiente neuronal. Aunque el porvenir de la metformina parece ser prometedor, la evidencia clínica no es concluyente para su uso en el cáncer o las enfermedades neurodegenerativas; aunque existen esperanzas de ampliar sus indicaciones en un futuro no tan lejano.
