El mayor éxito de la atención en las unidades de cuidados intensivos neonatales es alcanzar el alta del paciente con un aceptable pronóstico neurológico, actualmente se ha disminuído la mortalidad, pero no se han mejorado las secuelas en el neurodesarrollo. Los factores de riesgo perinatal pueden afectar distintamente en cada población. Se presenta un estudio caso-control de las bases de datos del Sistema Informático Perinatal compilado con los registros de consulta externa, en lactantes que presentaron afectación del neurodesarrollo, en el Hospital Gineco-Obstétrico Isidro Ayora, periodo 2012- 2016. Se realizó análisis relacional y multivariado, mediante modelos de regresión logística; explotando su utilidad estimativa y predictiva. Las condiciones que se asocian a neurodesarrollo alterado son: Apgar bajo, hospitalización prolongada, hipoxia perinatal, peso menor a 1500gr, prematurez moderada, y defectos congénitos. El nivel predictivo del modelo fue de 97,5% y el área bajo la curva ROC 0.81 (IC 0.76-0.85). Cuando estos factores de riesgo están presentes se puede predecir un resultado desfavorable en alrededor del 31% de casos. Es importante destacar, que en esta población la edad de la madre no presentó asociación significativa con alteración en el neurodesarrollo, tomando en cuenta la alta prevalencia de embarazos en adolescentes.
