Hemorajik İnme ve Agresif Antihipertansif Tedavi

Abstract

Acil servislerde hepimizin sıklıkla karşılaştığımız ve hızlı tanı-tedavi sürecini yönettiğimiz hasta gruplarından bir tanesi hemorajik inmelerdir. Bildiğimiz üzere, hemorajik inmeler, tüm inmelerin %10-20’sini oluşturmaktadır. İnme vakalarının %50’si mortal seyretmektedir. Mortalite ile sonuçlanmayan inmelerin ise ciddi bir bölümü sekelli iyileşmektedir. Hiç şüphesiz, mortalite ve morbiditesi bu kadar yüksek olan bu vakalarda,tedavinin etkin ve hızlı bir şekilde verilmesi hayati önem taşımaktadır. İntraserebral hemorajilerde; akut hipertansif yanıt sıklıkla görülmektedir ve bu yanıtı, mevcut hematomun genişlemesiyle ve artan mortalite ile ilişkilendirmek mümkündür. Acil servislerde, hemorajik inme tedavisinin en önemli basamağı kanamanın kontrol altına alınması olsa gerek. Kanama kontrolünün en önemli basamağı ise kan basıncı (KB) kontrolü. Kan basıncının kontrol altında tutulmasının, hematomun hacmini azaltacağını ve dolayısıyla mortaliteyi de azaltacağını söylemek mümkün. Konu ile alakalı kılavuzların önerileri doğrultusunda acil servislerde hemorajik inmeli hastalarda SKB<180mmHgolacak şekilde KB regülasyonu yapmaktayız.
