Evaluación de la salud bucal a través de la Teorı́a de la respuesta al Ítem en un estudio poblacional en Uruguay

Abstract

En los estudios epidemiológicos es práctica habitual trabajar con variables binarias que reflejan la presencia de determinadas enfermedades, las que a su vez pueden estar asociadas con otro conjunto de variables, que en general se asumen como factores de riesgo de las primeras. En el ámbito de los estudios epidemiológicos referidos a la salud bucal, es común indagar sobre la relación entre la presencia de algunas patologı́as y ciertas caracterı́sticas de los participantes del estudio a través de Modelos lineales generalizados (generalmente regresión logı́stica). No obstante ese tipo de análisis suele ser llevado a cabo de manera separada para cada variable de interés y en ningún momento se obtiene una medida que resuma el estado de cada participante. En este estudio se propone el uso de modelos de Teorı́a de Respuesta al Ítem (TRI) (especı́ficamente el modelo de Rasch) ya que estos permiten el análisis conjunto de varias variables obteniendo como subproducto una valoración individual, que en este caso se interpreta como "sickness proneness" (propensión a la enfermedad). Adicionalmente, el análisis presentado aquı́ extiende el modelo de Rasch incluyendo un predictor lineal que permite indagar sobre algunos los factores determinantes de la tendencia de los individuos a padecer las distintas patologı́as. Los resultados de este estudio confirman el efecto de la falta de actividad fı́sica y el avance de la edad sobre la disposición a presentar enfermedades bucales.
