Abstract

The HYPER-V 2.0 Protocol presents an integrated, evidence-based clinical framework for managing patients with clinical hypervigilance in reproductive medicine, oncofertility, and chronic pelvic pain (CPP). Hypervigilance — defined as a pathological state of heightened alertness to potential threats — is a significant but under-studied factor that worsens treatment outcomes in assisted reproductive technologies (ART), fertility preservation, and endometriosis care. This protocol integratesneurobiological mechanisms of central sensitization, polyvagal theory, biopsychosocial models, and cognitive-behavioral interventions with proprietary author-developed methods, including the HYPERBreath technique, Body Safety Mapping, and the Bridge Protocol for long-term patient support. The five-phase algorithm (Assessment → Regulation → Integration → Support → Validation) provides clinicians with a structured, reproducible approach to identifying, evaluating, and treating hypervigilance, while simultaneously debunking widespread myths of self-diagnosis that amplify patient distress.
