Abstract

Resumen La senescencia celular es un mecanismo protector frente al cáncer y al daño tisular, pero su acumulación progresiva con la edad contribuye a la inflamación crónica de bajo grado y a la patología asociada al envejecimiento. [ESTABLECIDO] El consenso actual sitúa la senescencia celular como uno de los doce sellos distintivos del envejecimiento, con un papel dual: beneficioso en la juventud (supresión tumoral, cicatrización) y perjudicial cuando las células senescentes no se eliminan eficazmente con la edad. [PROPUESTA CONCEPTUAL] Este trabajo plantea que el principal cuello de botella tecnológico no es la falta de fármacos senolíticos ni de biomarcadores de senescencia por separado, sino la ausencia de sistemas que acoplen ambas funciones —detección y eliminación— bajo un único mecanismo de activación, minimizando así la toxicidad fuera de objetivo y permitiendo una administración guiada por la carga senescente real del paciente. Se revisa la literatura existente sobre biomarcadores de senescencia, fármacos senolíticos y, en particular, los sistemas de profármacos activados por la enzima lisosomal beta-galactosidasa asociada a senescencia (SA-β-gal), que ya demuestran de forma natural el principio de acoplamiento detección-acción. A partir de esta base empírica, se propone una arquitectura conceptual de sistema integrado y se exponen sus limitaciones, riesgos regulatorios y una hoja de ruta de validación.
