Abstract

Obiettivo. Valutazione dei risultati sul dolore articolare e periarticolare del ginocchio, mediante una riflessoterapia con anestetici locali denominata anche neuralterapia.Disegno sperimentale. Studio retrospettivo osservazionale con follow-up di 3 anni.Ambiente. Centro Ambulatoriale di Terapia Antal-gica.Pazienti. Tutti i pazienti (N=115) con dolore al ginocchio trattati con la metodica in oggetto dal 1982 al 30-8-1996. I pazienti ritirati sono stati 2, e nei restanti 113 l’età media è stata di 68±15,2 anni.Interventi. La riflessoterapia impiegata, denominata anche neuralterapia secondo Huneke, utilizza anestetici locali (prevalentemente lidocaina) a concentrazione 0,5-1% ed in piccole quantità (0,5-1 cc per punto) nei punti tender/trigger e/o di agopuntura, nonché in strutture anatomiche come articolazioni, plessi nervosi e vasi arteriosi o venosi. Il numero medio di sedute è stato di 7,03±3,3.Rilevamenti. I risultati sono valutati in termini di miglioramento percentuale soggettivo da parte del paziente, al termine della terapia e ai follow-up successivi, e suddivisi in 5 gruppi (risultato nessuno, discreto, buono, molto buono, ottimo).Risultati. Al termine della terapia il totale dei casi migliorati è stato del 91,2%, dell’83,7% dopo 1 mese, del 64,6% dopo 3 mesi, del 41,3% dopo 6 mesi, del 22,5% dopo 1 anno, del 12,1% dopo 2 anni e del 7,9% dopo 3 anni.Conclusioni. In attesa di studi prospettici, randomizzati e in doppio cieco, i risultati ottenuti suggeriscono una buona efficacia antalgica della metodica proposta nelle patologie dolorose del ginocchio. Assessment of the outcomes on the articular and periarticular pain of the knee, by a reflexotherapy with local anesthetics, denominated also neuraltherapy. Retrospective observational study with 3 years' follow-up. An Ambulatory Center for Pain Management. All the patients (N = 115) with knee pain treated with neuraltherapy in the period 1982-31 August 1996 (retired patients 2, mean age 68 +/- 15.2 years in the remaining). The employed reflexotherapy, denominated also neuraltherapy by Huneke, uses a local anesthetic (prevalently lidocaine) in 0.5-1% concentration and small quantity (0.5-1 ml for point) on tender/trigger and/or acupuncture points, as well as in anatomical structures like articulations, nerves, arteries or veins. The number of sessions has been of 7.03 +/- 3.3. Outcomes are evaluated in terms of percentage subjective improvement of the pain at the end of the therapy and at 1-3-6 months, 1-2-3 years, and then divided in 5 groups (no improvement-fair-good-very good-excellent). At the end of the therapy the improven cases were the 91.2%, the 83.7% at 1 month, the 64.6% at 3 months, the 41.3% at 6 months, the 22.5% at 1 year, the 12.1% at 2 years and the 7.9% at 3 years. These outcomes suggest a good effectiveness of this treatment in the painful diseases of the knee, but further randomised double-blind trials are necessary.
