DIAGNOSIS DAN TATALAKSANA KATARAK SENILIS IMATUR PADA PETANI SAWIT USIA 67 TAHUN

Abstract

Cataracts are a leading cause of visual impairment and blindness worldwide, particularly in the elderly. Senile cataracts occur due to a progressive degenerative process of the lens that occurs with age. Exposure to ultraviolet light during outdoor work is known to be an additional risk factor, but not all cases of cataracts in field workers can be classified as occupational diseases. This case report aims to describe the diagnosis and management of immature senile cataracts in an elderly oil palm farmer. A 67-year-old man, a retired oil palm farmer, presented with complaints of blurred and cloudy vision in both eyes for the past year and worsening. This complaint was accompanied by glare when exposed to bright light. An ophthalmological examination revealed decreased vision and cloudy lenses were found in both eyes with a positive shadow test. The diagnosis was made as immature senile cataracts in the right and left eyes (ODS). The patient had worked as an oil palm farmer for approximately 40 years with chronic sun exposure with minimal personal protective equipment. Based on the history and physical examination, a diagnosis of occupational disease could not be established because the criteria for an occupational disease were not met according to the occupational medicine approach. The patient's cataract was more appropriately categorized as senile cataract due to an age-related degenerative process exacerbated by work, namely chronic ultraviolet light exposure. The patient was scheduled to undergo phacoemulsification with intraocular lens (IOL) implantation.
