AVALIAÇÃO DA EFICÁCIA DE MOLDES ODONTOLÓGICOS PERSONALIZADOS NA MINIMIZAÇÃO DOS EFEITOS ADVERSOS DA RADIOTERAPIA EM PACIENTES COM CÂNCER DE CABEÇA E PESCOÇO

Abstract

Introdução: Os moldes odontológicos personalizados, utilizados na radioterapia, são responsáveis por proporcionar maior conforto ao paciente e minimizar os danos decorrentes do tratamento. Na radioterapia de cabeça e pescoço, destacam-se três principais dispositivos: a máscara termoplástica, o bolus e o abridor de boca, cada um com função específica, variando conforme a localização anatômica e a extensão tumoral. Metodologia: Este estudo caracteriza-se como uma revisão integrativa da literatura, baseada predominantemente em artigos científicos indexados que abordam o uso de moldes na radioterapia de cabeça e pescoço. A busca foi realizada nas bases de dados SciELO, EBSCO HOST e PubMed/MEDLINE, além de literatura complementar. Foram utilizados os descritores “moldes para radioterapia” e “moldes para radioterapia em cabeça e pescoço”, associados pelo operador booleano AND. Foram incluídos estudos publicados no período de 2011 a 2026, disponíveis na íntegra, nos idiomas português e inglês, incluindo literatura cinzenta, considerando a abrangência do tema. O processo de seleção dos estudos foi organizado por meio de um fluxograma, contemplando as etapas de identificação, triagem, elegibilidade e inclusão. Resultados: Após o processo de triagem, 18 estudos foram incluídos na análise final, conforme demonstrado no fluxograma. A síntese dos estudos evidenciou que os dispositivos intraorais são eficazes na redução dos efeitos adversos da radioterapia, além de contribuírem para maior precisão no direcionamento da radiação e melhor posicionamento do paciente. Discussão: A utilização dos moldes está diretamente relacionada à localização da lesão e à necessidade de preservação dos tecidos adjacentes, sendo que esses dispositivos auxiliam no controle da radiação, na imobilização do paciente e na proteção de estruturas saudáveis, contribuindo para a redução de complicações e melhora da qualidade de vida. Conclusão: Diante do exposto, os moldes odontológicos personalizados mostram-se essenciais na radioterapia de cabeça e pescoço, contribuindo significativamente para a proteção dos tecidos saudáveis e para a redução dos efeitos adversos do tratamento. Sua correta indicação, confecção e utilização proporcionam maior conforto, segurança e bem-estar ao paciente, além de otimizar os resultados terapêuticos.

Introduction: Customized dental molds used in radiotherapy are responsible for providing greater patient comfort and minimizing damage resulting from treatment. In head and neck radiotherapy, three main devices stand out: the thermoplastic mask, the bolus, and the mouth opener, each with a specific function, varying according to the anatomical location and tumor extent. Methodology: This study is characterized as an integrative literature review, based predominantly on indexed scientific articles that address the use of molds in head and neck radiotherapy. The search was conducted in the SciELO, EBSCO HOST, and PubMed/MEDLINE databases, in addition to complementary literature. The descriptors "radiotherapy molds" and "head and neck radiotherapy molds" were used, associated by the Boolean operator AND. Studies published between 2011 and 2026, available in full text, in Portuguese and English, including grey literature, considering the breadth of the topic, were included. The study selection process was organized using a flowchart, encompassing the stages of identification, screening, eligibility, and inclusion. Results: After the screening process, 18 studies were included in the final analysis, as shown in the flowchart. The synthesis of the studies showed that intraoral devices are effective in reducing the adverse effects of radiotherapy, in addition to contributing to greater precision in radiation targeting and better patient positioning. Discussion: The use of molds is directly related to the location of the lesion and the need to preserve adjacent tissues. These devices assist in radiation control, patient immobilization, and protection of healthy structures, contributing to the reduction of complications and improved quality of life. Conclusion: In light of the above, customized dental molds are essential in head and neck radiotherapy, significantly contributing to the protection of healthy tissues and the reduction of adverse treatment effects. Their correct indication, fabrication, and use provide greater comfort, safety, and well-being to the patient, as well as optimizing therapeutic results.
