Abstract

"Workplace Accident Investigation" is a chapter from the e-book Técnicas de Segurança do Trabalho (Occupational Safety Techniques), authored by Rui A. C. Veiga (Merituscalabis, 2024). The chapter presents a structured, technical approach to investigating workplace accidents and near-misses. It covers the full investigation cycle: emergency response, evidence preservation, data collection, data organisation and analysis. Four investigation methods are examined in depth — SCAT (Systematic Cause Analysis Technique), the Five Whys, Root Cause Analysis (RCA), and the Ishikawa (fishbone) diagram. The text emphasises that investigations aim not to assign blame, but to identify root causes and prevent recurrence, strengthening occupational health and safety management systems.
