Abstract

L’aspect du sujet abordé dans cette étude. Cette étude mixte a comparé la qualité des soins avant et après la mise en œuvre du FBP dans la Zone de santé de Gemena (Sud‑Ubangi) et a identifié les facteurs contextuels qui limitent ou favorisent son efficacité. Ce que cette étude apporte de nouveau. Le FBP a amélioré six dimensions de la qualité sur sept, avec des gains notables pour l’efficacité (+20 %), la sécurité (+19 %) et la rapidité (+21 %). L’équité n’a pas progressé significativement. Le respect des indicateurs contractuels (OR = 2,71) et la régularité des incitations (OR = 2,45) sont les principaux moteurs. Les freins sont les retards de paiement (72 %), le manque d’équipements (68 %) et des montants incitatifs jugés insuffisants (65 %).
