TROMBECTOMIA MECÂNICA EM AVC ISQUÊMICO: REVISÃO INTEGRATIVA E ASPECTOS EPIDEMIOLÓGICOS

Apresenta-se por déficits neurológicos focais, como hemiparesia, afasia e ataxia. O diagnóstico é clínico-radiológico, sendo a ressonância magnética o exame de escolha. Além da trombólise, a trombectomia mecânica tem papel fundamental nas primeiras 24 horas de sintomas, especialmente quando o trombolítico não promove recanalização completa. Objetivo: Analisar aspectos epidemiológicos e a necessidade do uso da trombectomia mecânica em casos de AVCi. Metodologia: Revisão integrativa de literatura nas bases SciELO e PubMed, utilizando os descritores "AVC", "cateterismo", "trombectomia mecânica" e "epidemiologia". Foram identificados 303 artigos, dos quais cinco atenderam aos critérios de inclusão, abordando morbimortalidade, incidência, faixa etária, tempo de internação, taxa de recanalização e letalidade. Resultados: A aplicação da trombectomia mecânica permanece limitada, embora crescente com o avanço tecnológico e o reconhecimento do tratamento precoce. O acesso é restrito, sobretudo na rede pública. Quando realizada em até oito horas após o início dos sintomas, aumenta em cerca de 50% as chances de recuperação. Nos estudos analisados, 54,9% dos pacientes eram homens, com idade média de 64 anos e predomínio entre 60 e 79 anos. A taxa média de recanalização bem-sucedida foi de 71% (mTICI 2b/3). Pacientes tratados com stent-retrievers apresentaram menor morbimortalidade (29,2%) em comparação aos métodos clássicos (55,6%). O tempo médio de internação variou entre 48 horas e oito dias. Discussão: Apesar dos avanços, o acesso desigual à trombectomia e o custo elevado ainda limitam sua ampla aplicação. A eficácia do procedimento está diretamente associada à rapidez da intervenção e à estrutura hospitalar disponível, sendo essencial ampliar sua oferta e capacitação profissional. Conclusão: Diante da elevada carga de morbimortalidade do AVCi, é imprescindível expandir o uso da trombectomia mecânica, promover políticas de acesso equitativo e difundir o conhecimento epidemiológico sobre seu impacto clínico e prognóstico.
