Abstract

Resumen:Introducción y objetivo: La salud, la capacidad económica y los procesos cognitivos de internalización cultural suelen abordarse como dominios separados, tanto en la investigación como en la intervención pública. Sin embargo, en la experiencia del individuo, estas dimensiones operan como un sistema acoplado, cuyas interacciones determinan trayectorias de bienestar, productividad y vulnerabilidad. El presente artículo tiene como objetivo desarrollar los fundamentos teóricos y metodológicos para la construcción de tres índices compuestos —el Índice de Capital Económico Individual (ICEI), el Índice de Salud Anatómica (ISA) y el Índice de Internalización Cognitiva (IIC)— diseñados para operacionalizar estas dimensiones y permitir su análisis conjunto en un espacio ternario.Métodos: Se presenta una síntesis de marcos teóricos provenientes de la economía de la salud (teoría del capital humano), la antropometría funcional y la neurociencia afectiva y cognitiva. Para cada índice se definen: (1) su fundamentación conceptual, (2) las dimensiones que lo componen, (3) los indicadores específicos y su definición operativa, y (4) un método de agregación y estandarización. El ICEI integra indicadores de acceso a servicios preventivos, control de enfermedades crónicas, cobertura de vacunación y días de incapacidad laboral. El ISA combina medidas antropométricas (IMC, circunferencia de cintura, relación cintura-cadera, porcentaje de grasa corporal) y hemodinámicas (presión arterial). El IIC propone una métrica basada en resonancia magnética funcional (fMRI) que cuantifica la internalización de conceptos a partir de la actividad en corteza prefrontal ventromedial, amígdala y corteza cingulada anterior dorsal, integrando análisis de magnitud de señal, coeficientes de variación temporal y análisis de patrones multivoxel (MVPA). Se describe el procedimiento de estandarización intra-sujeto y la ecuación de agregación propuesta.Resultados: El marco metodológico presentado permite la construcción de los tres índices a nivel individual y su posterior agregación poblacional. Mediante un proceso de normalización que fuerza la suma constante, los índices pueden representarse conjuntamente en un diagrama ternario. Este espacio de representación hace visible la distribución de perfiles poblacionales y permite identificar tipologías de individuos en función del equilibrio entre su capital económico, su salud anatómica y su grado de internalización cognitiva de conceptos culturalmente relevantes. Se discuten, a modo de ejemplo, los perfiles extremos y sus potenciales implicaciones.Conclusiones: El marco ICEI-ISA-IIC constituye una propuesta conceptual y metodológica en etapa de desarrollo que requiere validación empírica. Su principal contribución es ofrecer un lenguaje integrado y una arquitectura metodológica que permite abordar fenómenos complejos desde una perspectiva multidisciplinar, superando la fragmentación tradicional de estas dimensiones. Se sientan así las bases para futuras investigaciones empíricas y aplicaciones en salud pública, planificación estratégica y estudio de dinámicas poblacionales. Mi visión es la siguiente: El marco propuesto aborda dos desafíos críticos en los sistemas de salud contemporáneos: la falta de adherencia terapéutica y el fenómeno de las "puertas giratorias": pacientes con enfermedades crónicas que acuden con frecuencia a los servicios de urgencias y atención primaria sin lograr una mejoría clínica, consumiendo así una parte desproporcionada de los recursos de salud pública. Al integrar la dimensión neurocognitiva (CII) con indicadores económicos y anatómicos, este modelo busca identificar las barreras implícitas —como la baja tolerancia al distrés, los sesgos de evitación y la internalización disfuncional de conceptos relacionados con la salud— que subyacen a la resistencia al tratamiento y perpetúan ciclos de atención de alto costo y baja efectividad. Por lo tanto, la representación ternaria ofrece una herramienta no solo para la estratificación poblacional, sino también para comprender por qué las intervenciones convencionales fallan en subgrupos específicos, allanando el camino para estrategias de salud pública precisas y adaptadas al perfil neuroafectivo del paciente. --- Abstract: Introduction and objective: Health, economic capacity, and the cognitive processes of cultural internalization are typically addressed as separate domains in both research and public intervention. However, in individual experience, these dimensions operate as a coupled system whose interactions determine trajectories of well-being, productivity, and vulnerability. This article aims to develop the theoretical and methodological foundations for constructing three composite indices—the Individual Economic Capital Index (ICEI), the Anatomical Health Index (ISA), and the Cognitive Internalization Index (IIC)—designed to operationalize these dimensions and enable their joint analysis within a ternary space. Methods: A synthesis of theoretical frameworks from health economics (human capital theory), functional anthropometry, and affective and cognitive neuroscience is presented. For each index, the following are defined: (1) its conceptual foundation, (2) its component dimensions, (3) specific indicators and their operational definitions, and (4) an aggregation and standardization method. The ICEI integrates indicators of access to preventive services, chronic disease control, vaccination coverage, and days of work disability. The ISA combines anthropometric (BMI, waist circumference, waist-to-hip ratio, body fat percentage) and hemodynamic (blood pressure) measures. The IIC proposes a metric based on functional magnetic resonance imaging (fMRI) that quantifies concept internalization through activity in the ventromedial prefrontal cortex, amygdala, and dorsal anterior cingulate cortex, integrating signal magnitude analysis, temporal variation coefficients, and multi-voxel pattern analysis (MVPA). The intra-subject standardization procedure and the proposed aggregation equation are described. Results: The presented methodological framework enables the construction of the three indices at the individual level and their subsequent population aggregation. Through a normalization process that forces constant sum, the indices can be jointly represented in a ternary diagram. This representational space visualizes the distribution of population profiles and allows identification of individual typologies based on the balance among their economic capital, anatomical health, and degree of cognitive internalization of culturally relevant concepts. Extreme profiles and their potential implications are discussed as examples. Conclusions: The ICEI-ISA-IIC framework constitutes a conceptual and methodological proposal in the development stage that requires empirical validation. Its main contribution is to offer an integrated language and methodological architecture that enables addressing complex phenomena from a multidisciplinary perspective, overcoming the traditional fragmentation of these dimensions. This lays the groundwork for future empirical research and applications in public health, strategic planning, and the study of population dynamics. My vision is as follows: The proposed framework addresses two critical challenges in contemporary healthcare systems: poor treatment adherence and the "revolving door" phenomenon: patients with chronic illnesses who frequently visit emergency and primary care services without achieving clinical improvement, thus consuming a disproportionate share of public health resources. By integrating the neurocognitive dimension (IIC) with economic and anatomical indicators, this model seeks to identify implicit barriers—such as low distress tolerance, avoidance biases, and dysfunctional internalization of health-related concepts—that underlie treatment resistance and perpetuate cycles of high-cost, low-effectiveness care. Therefore, the ternary representation offers a tool not only for population stratification, but also for understanding why conventional interventions fail in specific subgroups, paving the way for precise public health strategies tailored to the patient's neuroaffective profile.
