Quand un Syndrome Coronarien Mime une Myocardite : À Propos d'un Cas à Libreville (Gabon)

Abstract

La myocardite aiguë est un diagnostic différentiel majeur du syndrome coronarien aigu (SCA), en particulier chez le sujet jeune. Nous rapportons le cas d’un homme de 38 ans, obèse, admis pour douleur thoracique constrictive avec sus-décalage du segment ST inférieur et élévation massive de la troponine (4953 ng/L). Devant l’absence de nécrose à l’ECG et un contexte pseudo-infectieux, le diagnostic de SCA a été réévalué. La coronarographie, normale, a éliminé une lésion obstructive. L’IRM cardiaque a confirmé une myocardite aiguë par la présence d’un rehaussement tardif épicardique et d’un œdème. Ce cas souligne la nécessité de considérer la myocardite devant un SCA à coronaire saine, surtout lorsque l’IRM, examen clé, est difficile d’accès.
