НЕВРОЛОГИЧЕСКАЯ ДИПЛОПИЯ: КЛИНИКО-ДИАГНОСТИЧЕСКИЙ АЛГОРИТМ, ДИФФЕРЕНЦИАЛЬНАЯ ДИАГНОСТИКА И СОВРЕМЕННЫЕ ПОДХОДЫ К ЛЕЧЕНИЮ

Abstract

Резюме. Неврологическая диплопия является клинически значимым симптомом, отражающим поражение глазодвигательных нервов, стволовых структур головного мозга либо системных микроваскулярных и демиелинизирующих процессов. Ошибки первичной интерпретации диплопии нередко приводят к гипердиагностике инсульта либо, напротив, к пропуску потенциально угрожающих состояний. В статье представлен систематизированный клинико-диагностический алгоритм ведения пациентов с неврологической диплопией, рассмотрены ключевые аспекты дифференциальной диагностики, а также современные (2024–2025 гг.) подходы к этиопатогенетической терапии. Приведён клинический случай с данными магнитно-резонансной томографии, иллюстрирующий практическую значимость алгоритмического подхода. Ключевые слова: неврологическая диплопия, глазодвигательные нервы, микроваскулярная нейропатия, МРТ, клинический алгоритм, дифференциальная диагностика.
