Varicella Zoster Virus in childhood is a frequent self-limiting pathology with a benign outcome. Although the incidence has dramatically reduced after immunization programs, outbreaks still occur. Neurologic complications related to this disease are rare. Arterial ischemic stroke and varicella is a known but rare association. The pathophysiology remains unclear and no standard protocol in the treatment. We report the case of a thirteen years old black girl who had been immunize against varicella zoster during infancy. She developed varicella four weeks prior to an abrupt onset of a left sided weakness. Imaging results revealed an infarct in the right basal ganglia. Biological exams were conclusive in the etiology of varicella. Our management included one-month oral acyclovir and physiotherapy with good outcome and complete recovery of the motor deficit.
