Abstract

Les plaies cervicales auto-infligées représentent des urgences chirurgicales et psychiatriques complexes. Nous rapportons deux cas pris en charge en 2023 mettant en lumière ce défi. Un cadre de 45 ans, anxieux suite à une rétrogradation professionnelle et récemment diagnostiqué VIH+, présente une plaie de 7 cm au-dessus du cartilage thyroïde. L'exploration révèle des sections de l'épiglotte, de la membrane thyro-épiglottique et du cartilage thyroïde. Un homme de 28 ans, amputé et en situation de rejet familial après un décès, présente une plaie de 6 cm exposant l'épiglotte. Les deux patients ont bénéficié d'une chirurgie réparatrice, d'une antibiothérapie et d'un soutien psychologique. Le premier est sorti contre avis médical au 2e jour, le second a développé une suppuration liée à son indigence avant une sortie à J11. Ces cas illustrent l'importance cruciale d'une approche intégrant chirurgie, psychiatrie et soutien socio-économique pour ces patients vulnérables.
