DIABET VA METABOLIK SINDROMDA LEYKOTSIT "PRIMINGI" ENDOTELIY GLIKOKALIKSI, KAPILLYAR RAREFAKSIYA VA PERIFERIK ANGIOPATIYA

Abstract

Diabetes mellitus va metabolik sindrom butun dunyo boʻylab tarqalgan muhim sogʻliq muammosiga aylangan. Kasallikning asosiy ogʻir asoratlaridan biri mikrovaskulyar va makrovaskulyar komplikatsiyalar, jumladan, periferik angiopatiyadir. Soʻnggi yillarda, diabetning vaskulyar asoratlarining patogenezida yalligʻlanish jarayonlarining, xususan, leykotsitlarning faollashuvi yoki “priming” jarayonining roli keng oʻrganilmoqda. Ushbu sharhli maqola 2015-2025 yillar oraligʻida chop etilgan ilmiy ma'lumotlarga asoslanib, diabet va metabolik sindrom sharoitida leykotsit primingi, endoteliy glikokaliksning buzilishi, kapillyar rarefaksiya va periferik angiopatiya oʻrtasidagi kompleks oʻzaro bogʻliqlikni tahlil qiladi. Endoteliy glikokaliks – bu endotelial hujayralarning ichki qavatini qoplaydigan, vaskulyar homeostazni saqlashda muhim rol oʻynaydigan murakkab glikoprotein va glikozaminoglikan qatlami. Diabet sharoitida giperglikemiya, insulin rezistentligi va oksidativ stress tufayli endoteliy glikokaliksning degradatsiyasi yuz beradi, bu esa leykotsitlarning endoteliyga yopishishi va transendotelial migratsiyasini osonlashtiradi. Leykotsit primingi ularning migratsiyasi, sitokin ishlab chiqarishi va oksidativ moddalar generatsiyasini kuchaytiradi, bu esa mahalliy yalligʻlanishni kuchaytiradi va endotelial disfunksiyaga olib keladi. Uzoq muddatli yalligʻlanish va mikrovaskulyar stress natijasida kapillyar tarmoqlarning rarefaksiyasi, ya'ni mikrovaskulyar zichlikning pasayishi roʻy beradi, bu esa toʻqimalarning kislorod va ozuqa moddalari bilan ta'minlanishini yomonlashtiradi. Kapillyar rarefaksiya va periferik qon tomir disfunksiyasi birgalikda asosiy klinik koʻrinish boʻlgan periferik angiopatiyaning rivojlanishiga hissa qoʻshadi. Ushbu maqolada leykotsit primingini modulyatsiya qilish, endoteliy glikokaliksni tiklash va mikrovaskulyar zichlikni saqlashga qaratilgan yangi terapevtik strategiyalar ham muhokama qilinadi. Bu yondashuvlar diabet bilan ogʻrigan bemorlarda vaskulyar asoratlarning oldini olish va ularni davolash yangi yoʻnalishlarini ochishi mumkin. Tadqiqotlar shuni koʻrsatadiki, glikokaliksni himoya qiluvchi vositalar, antiyalligʻlanish terapiyasi va yangi qon tomirlarining oʻsishini ragʻbatlantiruvchi (angiogenez) omillar potentsial terapevtik strategiyalar sifatida e'tiborga loyiqdir.
