Abstract

This technical report provides a documentary and operational risk analysis of contingency management in the vaccine cold chain, focusing on power supply interruptions and refrigeration equipment failures. It examines a revised Standard Operating Procedure (SOP) implemented in the Central Health Region of the Federal District (Brazil), identifying vulnerabilities that threaten vaccine thermal stability and service continuity. The report details a standardized rapid-response workflow, including ice-pack conditioning, correct thermal-container preparation, continuous temperature monitoring within +2°C to +8°C, and operational criteria for safe stock transfer to conditioned containers or backup refrigeration. It also describes coordination steps with facility security and the power utility to reduce response time and prevent avoidable losses. By translating routine steps into a structured, replicable contingency model, this report supports more resilient immunization services, helping protect vaccine potency, reduce waste, and sustain program performance during adverse events. The model is intended to be adaptable across immunization settings, including resource-constrained clinics and public health networks. The analysis is restricted to the SOP content and involves no clinical interventions or identifiable personal patient data.
