Abstract

Resumen Introducción: la práctica de la enfermería geriátrica enfrenta tensiones éticas derivadas de la fragilidad, la dependencia y la institucionalización de la persona adulta mayor. La ética ontológica del cuidado ofrece un marco teórico-conceptual que reconoce a la persona como sujeto moral. Objetivo: analizar, desde la perspectiva de la ética ontológica del cuidado, el concepto de dignidad humana y la tensión ética entre autonomía y vulnerabilidad en el contexto de la práctica de la enfermería geriátrica. Metodología: revisión documental crítica de 26 fuentes académicas (2020–2025), en español e inglés, seleccionadas por su aporte conceptual en ética del cuidado, dignidad, autonomía relacional y vulnerabilidad. La información se organizó en categorías temáticas y se realizó un análisis narrativo-crítico, integrando categorías filosóficas, bioéticas y propias de la enfermería. Resultados: la praxis de la enfermería se configura como un espacio ético en el que convergen la dignidad, la autonomía relacional y la comprensión de la vulnerabilidad. Para garantizar un cuidado respetuoso y ético, es crucial distinguir entre vulnerabilidad ontológica y fragilidad clínica. Conclusión: para orientar la formación profesional, la práctica clínica y las políticas institucionales, la ética ontológica del cuidado constituye un marco transformador al contribuir a la humanización de la enfermería geriátrica y a la preservación de la dignidad y la responsabilidad moral tanto del profesional de la salud como de la persona cuidada.

Introduction: Geriatric nursing practice faces ethical tensions stemming from the frailty, dependency, and institutionalization of older adults. The ontological ethics of care offers a theoretical and conceptual framework that recognizes the person as a moral subject. Objective: To analyze, from the perspective of the ontological ethics of care, the concept of human dignity and the ethical tension between autonomy and vulnerability in the context of geriatric nursing practice. Methodology: A critical literature review of 26 academic sources (2020–2025) in Spanish and English was conducted, selected for their conceptual contributions to the ethics of care, dignity, relational autonomy, and vulnerability. The information was organized into thematic categories, and a narrative-critical analysis was performed, integrating philosophical, bioethical, and nursing categories. Results: Nursing practice is configured as an ethical space, characterized by dignity, relational autonomy, and an understanding of vulnerability. To guarantee respectful and ethical care, it is crucial to distinguish between ontological vulnerability and clinical frailty. Conclusion: To guide professional training, clinical practice, and institutional policies, the ontological ethics of care constitutes a transformative framework for humanizing geriatric nursing practice and preserving the dignity and moral responsibility of both the healthcare professional and the patient.
