
A hemorragia intracerebral (HIC) espontânea é condição associada a alta mortalidade e incapacidade, exigindo abordagem integrada com controle pressórico, reversão de coagulopatias e cuidados neurointensivos. A indicação cirúrgica é mais estabelecida em hematomas cerebelares com deterioração neurológica, hidrocefalia ou compressão de tronco encefálico. Em HIC supratentorial, a craniotomia aberta rotineira não demonstrou benefício consistente em ensaios clássicos, o que estimulou o desenvolvimento de estratégias minimamente invasivas (aspiração/endoscopia e protocolos guiados). A decisão deve considerar localização, volume, tempo de evolução, idade, estado clínico e disponibilidade de recursos. Conclui-se que a cirurgia tem papel definido em subgrupos específicos e que abordagens minimamente invasivas podem ampliar opções terapêuticas em cenários selecionados.
