Aspectos neuropsicológicos e correlatos neurofuncionais da Síndrome da Covid Longa

Abstract

Resumo: O objetivo deste estudo foi revisar os aspectos neuropsicológicos da síndrome da Covid Longa, diferenciando as queixas subjetivas dos déficits cognitivos objetivamente verificados. A análise da literatura indica que, embora haja heterogeneidade, o perfil neurocognitivo predominante envolve prejuízos na atenção, nas funções executivas e na velocidade de processamento. Do ponto de vista fisiopatológico, evidências de neuroimagem funcional e biomarcadores sugerem que mecanismos de neuroinflamação sustentada, disfunção microvascular e hipometabolismo cerebral, notadamente em regiões do córtex cingulado e estruturas límbicas, constituem o substrato biológico dessas alterações. Conclui-se que a COVID Longa se comporta, em muitos casos, como uma encefalopatia pós-infecciosa, exigindo a inclusão sistemática de rastreio neuropsicológico e programas de reabilitação cognitiva nos protocolos de assistência pós-aguda. Abstract: This study reviews the neuropsychological aspects of the syndrome, differentiating subjective complaints from objectively verified cognitive deficits. The literature analysis indicates that, despite heterogeneity, the predominant neurocognitive profile involves impairments in attention, executive functions, and processing speed. From a pathophysiological perspective, evidence from functional neuroimaging and biomarkers suggests that mechanisms of sustained neuroinflammation, microvascular dysfunction, and cerebral hypometabolism—notably in regions of the cingulate cortex and limbic structures—constitute the biological substrate of these alterations. It is concluded that Long COVID behaves, in many cases, as a post-infectious encephalopathy, requiring the systematic inclusion of neuropsychological screening and cognitive rehabilitation programmes in post-acute care protocols..
