
This document is the official Post-Submission Authorship Amendment Form for academic journals (English/Persian). It is designed to formally request any changes to the author list—including additions, removals, or reordering of authors, or a change in the corresponding author—after a manuscript has been submitted but prior to its final publication. The structure and required content of this form are adapted from and inspired by the general authorship change request policy used by Elsevier, as outlined in their document available at: https://legacyfileshare.elsevier.com/gfa/authorship-change-request-form.pdf. The form requires detailed justification for all changes, adherence to ICMJE authorship criteria, disclosure of contributions using CRediT roles for new authors, and mandatory electronic or handwritten signatures from all parties involved (including those being removed) to ensure full consensus and transparency in the academic record.
