Cambios en el índice triglicéridos-glucosa tras un año de terapia antirretroviral

Abstract

Resumen Introducción: la terapia antirretroviral (TAR) es muy eficaz para tratar el VIH, pero se asocia a alteraciones metabólicas, como la resistencia a la insulina (RI), la cual puede evaluarse con el modelo homeostático HOMA-IR y alternativas más accesibles como el índice triglicéridos-glucosa (TyG), validado en población mexicana con un punto de corte de 4.68, sensibilidad de 96.5% y especificidad de 85%. Sin embargo, no hay estudios que analicen su aplicabilidad y cambios tras el inicio de TAR en pacientes con VIH. Objetivo: describir la frecuencia de RI usando el índice TyG en pacientes con VIH sin tratamiento previo (naive) antes y al año de la TAR. Material y métodos: estudio transversal comparativo. Se revisaron expedientes de Medicina Interna e Infectología de un hospital de tercer nivel de pacientes con diagnóstico reciente de VIH durante 2010-2025. Se recolectaron datos bioquímicos, clínicos y antropométricos basales y al año de tratamiento. Las variables se registraron y analizaron de manera cegada con el programa SPSS, v. 25, con estadística no paramétrica. Una p 4.68) del punto de corte para RI posterior a un año de TAR. En contraste, en aquellos que tuvieron un índice TyG elevado el 72% permaneció elevado.
