Abstract

Resumen Introducción: el micetoma es una infección crónica subcutánea causada por hongos (eumicetoma) o bacterias filamentosas (actinomicetoma), adquirida por inoculación traumática. Afecta la piel, el tejido subcutáneo y, en casos avanzados, estructuras profundas como huesos o la médula espinal. Su presentación puede derivar en complicaciones severas, entre las que se incluye la paraplejía. Caso clínico: paciente del sexo masculino de 74 años, agricultor, hospitalizado por paraplejía de forma súbita, con incapacidad para deambular e incontinencia urinaria y fecal. Se interconsultó por presentar dermatosis que afectaba el tronco, en la cara posterior del hemitórax izquierdo, localizada y asimétrica, la cual estuvo constituida por múltiples fístulas con salida de secreción purulenta, e incontables cicatrices atróficas eritemato-violáceas que confluían y formaban una placa de 30 x 30 cm. A nivel neurológico se integró síndrome medular completo. Se hicieron estudios microbiológicos en los que se observó la presencia de granos de entre 50 y 100 μm de bacterias filamentosas positivas a ácido peryódico de Schiff (PAS) y ácido-alcohol resistente con tinción de Ziehl-Neelsen, compatibles con Nocardia spp. La resonancia magnética demostró compromiso de T6-T7-T8. Conclusiones: el actinomicetoma es endémico en México. La triada clásica incluye masa subcutánea, fístulas y granos. Su progresión puede causar destrucción vertebral y mielopatía. La resonancia magnética es el estudio de elección por su alta resolución para tejidos blandos. El abordaje terapéutico combina antibióticos con actividad frente a actinomicetos.
