EXTRAVASAMENTO DE CLOREXIDINA DURANTE O TRATAMENTO ENDODONTICO: RELATO DE CASO

Abstract

RESUMO O extravasamento de soluções irrigadoras durante o tratamento endodôntico é uma intercorrência grave que pode comprometer a saúde do paciente. Entre os irrigantes, a clorexidina a 2% apresenta boas propriedades antimicrobianas e substantividade, porém, quando extravasada, pode causar dor intensa, edema, necrose e parestesia. Este trabalho relata um caso clínico de extravasamento de clorexidina, abordando a conduta terapêutica e a evolução clínica. O caso ocorreu em uma paciente de 48 anos, atendida na clínica escola da Faculdade Paulo Picanço, durante o tratamento endodôntico do dente 25. Durante a irrigação, houve extravasamento para os tecidos periapicais, resultando em dor imediata e edema. A conduta inicial incluiu medicação intracanal, antibióticos, corticoides, analgésicos e laserterapia de baixa intensidade. Nos dias seguintes, o edema aumentou, alcançando a região infrapalpebral, exigindo atendimento emergencial. O acompanhamento clínico, associado a sessões seriadas de laserterapia e citoneurin, proporcionou regressão do processo inflamatório e recuperação completa da sensibilidade. O caso reforça a importância da irrigação controlada, do uso de seringas com ponta romba e da verificação do comprimento de trabalho, destacando a eficácia da fotobiomodulação como recurso coadjuvante na modulação inflamatória e regeneração tecidual.
