СОВРЕМЕННЫЕ ХИРУРГИЧЕСКИЕ ПОДХОДЫ К ЛЕЧЕНИЮ ПРОЛАПСА ОРГАНОВ МАЛОГО ТАЗА

Abstract

Пролапс органов малого таза (ПОМТ) — распространённое заболевание среди женщин, особенно старше 50 лет, нередко сопровождающееся стрессовым недержанием мочи (СНМ). Основными хирургическими методами лечения являются передняя и задняя кольпорафия, лапароскопические вмешательства и установка подуретральных слингов (TVT, TOT). Несмотря на эффективность вмешательств, остаётся высокая частота рецидивов и осложнений, особенно при игнорировании индивидуальных анатомо-функциональных особенностей тазового дна. Установка ленты TVT показала высокую эффективность в лечении СНМ, особенно в сочетании с цистоцеле, по сравнению с изолированной кольпорафией. Современная тенденция — переход к персонализированным хирургическим подходам с учётом возраста, сопутствующих заболеваний и функционального состояния тазового дна.
