OBSTRUÇÃO DO TRATO URINÁRIO DE CÃES E GATOS

Abstract

A obstrução do trato urinário em cães e gatos é uma urgência frequente. Em gatos, a obstrução uretral ocorre quase exclusivamente em machos e decorre sobretudo de plugs uretrais associados à doença do trato urinário inferior felino/idiopática, além de urolitíase (principalmente estruvita e oxalato de cálcio) e, menos comuns, estenose/neoplasia. Em cães, a causa predominante é urolitíase uretral/vesical (estruvita, oxalato de cálcio; ainda, cistina/urato em raças predispostas), seguida por estenoses, neoplasias e doença prostática. O manejo requer estabilização (analgesia, correção hidroeletrolítica/ácido–base) e descompressão: cistocentese descompressiva conforme necessidade, cateterização com hidropropulsão retrógrada, e uro-hidropropulsão para remoção de cálculos vesicais selecionados. Dissolução dietético-antimicrobiana é opção para urólitos de estruvita em cães com infecção controlada. Procedimentos cirúrgicos incluem cistotomia, uretrostomias (perineal em gatos; escrotal em cães) para casos recorrentes/irreversíveis, e, no trato superior, as obstruções ureterais podem ser corrigidas por meio de microcirurgia com ou sem implante de stents ureterais (Duplo J) e casos onde já não se tenha patência ureteral há indicação do bypass ureteral subcutâneo (SUB). Tendências recentes mostram predomínio de estruvita e oxalato de cálcio entre urólitos mais comuns. O prognóstico é favorável quando há desobstrução oportuna e controle de recorrência (hidratação, dieta terapêutica, manejo ambiental em felinos).
