Intoxicación alimentaria por clenbuterol con hipokalemia moderada y rabdomiolisis temprana: reporte de un caso clínico

Abstract

{Introducción:} El clenbuterol es un agonista β2-adrenérgico utilizado ilegalmente para promover el crecimiento muscular en animales. Su consumo humano, generalmente a través de carne contaminada, puede producir una serie de efectos adversos cardiovasculares, metabólicos y musculares potencialmente graves. {Presentación del caso:} Mujer de 43 años ingresó al área de primer contacto con taquicardia de 133 lpm posterior al consumo de pollo contaminado en su lugar de trabajo. Los estudios iniciales reportaron hipokalemia moderada (K sérico de 2.69 mmol/L) y un electrocardiograma con taquicardia sinusal sin otras alteraciones. Fue manejada con reposición de potasio, betabloqueo con propranolol inicialmente, y posteriormente con metoprolol en fase de observación. A las 12 horas del ingreso, se detectó rabdomiolisis con CPK de 987 U/L, lo que motivó ajuste del manejo hídrico con soluciones cristaloides, aumentando el requerimiento a 1.5 veces la fórmula de Holliday-Segar. La paciente permaneció en observación en el servicio de urgencias durante 48 horas con monitoreo electrocardiográfico y evolución favorable. {Conclusiones:} La intoxicación por clenbuterol derivada del consumo de carne contaminada representa un problema de salud pública subestimado. Este caso subraya la necesidad de considerar esta etiología ante la presencia de taquicardia, hipokalemia y signos tempranos de rabdomiolisis sin otra causa aparente.
