PREECLAMPSIA MANEJO EN BASE A FACTORES ANGIOGENICOS

Abstract

La ponencia abordó el manejo de la preeclampsia basado en factores angiogénicos, destacando su impacto como segunda causa de mortalidad materna en Ecuador (25-38% de casos entre 2021-2025). Se presentaron datos epidemiológicos nacionales que muestran una reducción progresiva en muertes maternas (180 en 2020 vs. 86 en 2024), aunque persisten desafíos en provincias como Guayas y Pichincha. Fundamentado en evidencia de ensayos clínicos (HYPITAT, PHOENIX) y metaanálisis, se analizó el balance entre parto inmediato y manejo expectante: para preeclampsia leve >37 semanas, el parto planificado reduce complicaciones maternas (RR 0.86; IC 0.79-0.94), pero entre 34-36.6 semanas aumenta riesgos neonatales (admisión a UCI neonatal: RR 2.0). Como eje innovador, se enfatizó el rol de los biomarcadores angiogénicos (ratio *sFlt-1/PlGF*): Ratio 85 (antes de 34 semanas) o >110 (después de 34 semanas): Predice progresión a preeclampsia grave, indicando hospitalización, maduración pulmonar fetal y posible interrupción en 48 horas. Se presentaron algoritmos clínicos (aprobados por la FDA en 2023) que integran estos biomarcadores para decisiones de manejo ambulatorio/hospitalario, junto con estrategias de prevención (AAS antes de semana 16 en alto riesgo). Las conclusiones subrayaron: 1) Finalización obligatoria a las 37 semanas, 2) Decisión individualizada entre 34-36.6 semanas con consentimiento informado, y 3) Clasificación de preeclampsia en Tipo I (placentaria, temprana) y Tipo II (materna, tardía) para abordaje personalizado.
