Desafios diagnósticos en el cáncer de mama triple negativo. Presentación de un caso.

Abstract

El cáncer de mama es la 12° causa de muerte femenina en Ecuador. El cáncer de mama triple negativo (TNBC), caracterizado por la ausencia de receptores de estrógeno (ER), progesterona (PR) y sobreexpresión de HER2, representa del 15 al 20% de todos los cánceres de mama. Este subtipo tiene un comportamiento agresivo con evolución rápida y suele manifestarse como nódulos o masas palpables en lugar de detectarse mediante tamizajes mamarios. Presenta un mal pronóstico, con una tasa de supervivencia a 5 años menor en comparación con otros subtipos, y afecta especialmente a pacientes jóvenes. Además, el TNBC puede imitar condiciones benignas debido a sus características imagenológicas, lo que puede llevar a diagnósticos erróneos o tardíos. Esta heterogeneidad en su presentación clínica y radiológica subraya la necesidad de un enfoque diagnóstico integral que combine evaluación clínica, estudios de imagen avanzados (como mamografía, tomosíntesis y ecografía) y confirmación histopatológica con inmunohistoquímica. La detección oportuna y el manejo multidisciplinario son cruciales para mejorar los desenlaces en estas pacientes, quienes tienen opciones terapéuticas más limitadas debido a la falta de blancos hormonales. Este caso clínico destaca los desafíos diagnósticos del TNBC y la importancia de correlacionar hallazgos clínicos, radiológicos y patológicos para evitar retrasos en el tratamiento.
