Abstract

RESUMEN El coma mixedematoso es una afección patológica infrecuente que se desarrolla en personas cuya función tiroidea es prácticamente nula con síntomas característicos; la evolución clínica dependerá de un alto índice de sospecha y del inicio rápido del tratamiento. El cuadro típico es la letargia que progresa al estupor y finalmente al coma; debe insistirse que no necesariamente ocurre con un estado de coma en el paciente, por lo que sería más correcto llamarlo estado mixedematoso. El objetivo de este reporte es presentar el caso clínico de un varón de 42 años originario de la ciudad de Teziutlán que acude a Unidad Médica Nefro-Tez, el cual no tenía antecedentes patológicos importantes a excepción de una infección previa por SARS-CoV-2. Se realiza una revisión de la presentación clínica del caso, así como su manejo diagnóstico y terapéutico.

Myxedema coma is an infrequent pathological condition that develops in people whose thyroid function is practically non-existent. The clinical evolution will depend on a high index of suspicion and the prompt initiation of treatment. The typical picture is lethargy progressing to stupor and finally coma; It should be emphasized that myxedema coma does not necessarily occur with a coma state in the patient, so it would be more correct to call it a myxedema state. The objective of this report is to present the clinical case of a 42-year-old man from the city of Teziutlán who attends the Nefro-tez Medical Unit, who was unaware of previous pathologies, as the only relevant history, he refers to previous infection by SARS-CoV-2. A review of the clinical presentation of the case is carried out, as well as its diagnostic and therapeutic management.
