Abstract

The objective of this deliverable is (1) to timely present main recommendations to support development and implementation of C4D practices based on in-depth understanding of the VL best practice, and (2) to provide baseline understanding of each C4D practice contexts including key stakeholders. This deliverable addresses cross-sectional elements that will be reflected upon on communication and dissemination activities (D2.3); country implementation plans (D5.2), train-the trainers actions (D6.1), sustainability oriented actions (D4.1) and the reports on the Phase I and Phase II activities (D6.2 and D7.1). The intended audience are members from C4D teams from implementing countries, and other members from competent authorities and associated entities. In addition, main learnings are important to institutions, associations and other stakeholders outside the consortium of this Joint Action, interested in designing and implementing sustainable educational programmes for diabetes or other (chronic) diseases, or contemplate to apply the principle of good practice transfer in improving health and healthcare. State-of-the-art methodological frameworks and approaches were used to analyse implementation process (Consolidated Framework for Implementation Research CFIR), context characteristics (SCIROCCO Maturity Model), and to provide situation analysis of national/regional and local context (SWOT) including stakeholders’ analysis. Main results from in-depth analysis of VL best practice, relevant for the implementationprocess, are: (1) Intervention characteristics: VL best practice was internally developed with strong support of a wide variety of outside stakeholders, was unique to the Dutch context, and addressed the multidimensional nature of type 2 diabetes management;(2) Outer setting: Patient needs and views are continuously considered and the practice has adapted to meet the needs, VL is continuously engaged with stakeholders, and practice is aligned with national policies and funding frameworks; (3) Inner setting: The organizational structure supports continuous information flow between VL teams, frontline professionals and participants of the program, goals are clearly communicated, acted upon, and aligned with the staff, continuous learning and improvement are encouraged;(4) Characteristics of individuals: self-reflection in achieving implementation goals is valued and supported; passionate commitment of individuals to the goals of the organization exists and there is a perception that they have an impact on practice development: (5) Process: clear protocols exist, including the training; continuous efforts to identify and empower champions within and outside of the VL to overcome indifference or resistance, and there is continuous quantitative and qualitative feedback about progress and experience.
