Deintenzifikace inzulinoterapie u diabetes mellitus 2. typu v klinické praxi

Abstract

Potreba simplifikace (deintenzifikace) intenzifikovaneho inzulinoveho rezimu (IIR) je u pacientu s diabetes mellitus 2. typu dusledkem casteho zahajovani teto terapie v minulosti, kdy nebyly k dispozici jine alternativy. Deintenzifikace je v soucasne dobe umoznena dostupnosti fixnich kombinaci bazalniho inzulinu a agonistu receptoru glukagonu podobneho peptidu 1 (giukagon-iike peptide 1, GLP-1) (iGlarLixi, IDegLira). Studie IDEAL prokazala, ze deintenzifikace IIR prechodem na iGlarLixi je u pacientu s diabetes mellitus 2. typu ucinnou a bezpecnou moznosti simplifikace terapie, ktera poskytuje porovnatelnou glykemickou kompenzaci, benefity redukce telesne hmotnosti, snizeni mnozstvi inzulinovych injekci a celkove denni davky inzulinu, zlepseni hodnot pri kontinualnim monitorovani glykemie (continuous glucose monitoring, CGM), a to bez zvyseneho rizika hypoglykemie a s vyssi spokojenosti pacientu s lecbou.
