Tratamiento ortopédico con máscara facial y ortodoncia correctiva en paciente clase III esqueletal en el último pico de crecimiento

Abstract

La maloclusión Clase III esquelética plantea un desafío clínico que requiere intervención oportuna para evitar procedimientos invasivos como la cirugía ortognática. Este estudio tuvo como objetivo describir el manejo ortopédico con máscara facial y ortodoncia correctiva, evaluando su efectividad en una paciente de 12 años con Clase III en el último pico de crecimiento puberal. Se empleó un diseño cualitativo de caso único, analizando una adolescente con mordida cruzada posterior bilateral, overjet negativo, protrusión del incisivo inferior y proquelia labial, atendida en la Universidad de Guayaquil (2023-2025). La metodología incluyó una revisión bibliográfica exhaustiva de 115 fuentes científicas, análisis detallado de la historia clínica y evaluación de maduración ósea mediante vértebras cervicales con el método Baccetti. El tratamiento combinó expansor Hyrax y máscara facial (14 horas diarias, 4 meses), logrando corrección de la relación esquelética, mejora significativa del perfil facial, aumento del overjet, mejor proyección labial y armonía estética. Los resultados confirman que la intervención ortopédica, guiada por la maduración ósea, es efectiva incluso en etapas tardías del crecimiento puberal, destacando la importancia de un diagnóstico individualizado. La edad biológica, más que la cronológica, predice el éxito terapéutico. Concluimos que la combinación de expansión maxilar rápida y máscara facial es una estrategia altamente viable para corregir maloclusiones Clase III, optimizando resultados dentales, esqueléticos y funcionales, mejorando el bienestar emocional, estético, la calidad de vida, la confianza del paciente, su integración social y la funcionalidad oclusal, reduciendo la necesidad de tratamientos invasivos futuros.
