Depresivnost i anksioznost nakon COVID-19 i regionalna volumetrija mozga

Abstract

Uvod: COVID-19 može izazvati čitav opseg neuropsihijatrijskih simptoma neposredno nakon infekcije i u postakutnoj fazi. Dugotrajne sekvele u vidu depresivnosti i anksioznosti se mogu javiti i kod asimptomatskih SARS-CoV-2 infekcija i mogu biti povezane sa smanjenim volumenom supratentorijalnih moždanih regija. Cilj istraživanja je bio ispitati korelaciju depresivnosti i anksioznosti sa volumetrijom specifičnih regija mozga kod pacijenata 6-12 meseci nakon infekcije SARS-CoV-2. Metode: Istraživanje je obuhvatilo 88 pacijenata oba pola, 18-65 godina, koji su bili podeljeni u tri grupe u zavisnosti od težine kliničke slike akutne faze bolesti (blaga, umerena, teška). Primenjeni su Bekov inventar depresivnosti II (BDI II) i Skala za procenu depresivnosti, anksioznosti i stresa (DASS-21). Načinjen je MRI endokranijuma i pomoću platforme „volBrain” izračunati su volumeni moždanih regiona od značaja. Svi rezultati su predstavljeni u relativnim vrednostima u odnosu na ukupan intrakranijalni volumen. Rezultati: Kod pacijenata sa blagom kliničkom slikom utvrđena je statistički značajna korelacija (p<0,05) između BDI II i volumena desnog (ρ=0,431) i levog talamusa (ρ=0,43). Beleži se korelacija između DASS-21-A i volumena sive mase celog mozga (ρ=0,423), sive mase cerebruma (ρ=0,413) i desne amigdale (ρ=0,502). Kod pacijenata sa umerenom kliničkom slikom utvrđena je korelacija između DASS-21-D i volumena bele mase cerebruma (ρ=-0,424) i desnog talamusa (ρ=-0,421), korelacija između DASS-21-A i volumena bele mase cerebruma (ρ=-0,418) i korelacija ukupnog skora DASS-21 i volumena bele mase cerebruma (ρ=-0,468). Zaključak: Kod pacijenata sa blagom kliničkom slikom viši nivo depresivnosti je povezan sa većim volumenom oba talamusa, a viši nivo anksioznosti je praćen većim volumenima sive mase celog mozga, cerebruma i desne amigdale. Kod pacijenata sa umerenom kliničkom slikom depresivnost je korelirala sa volumenom bele mase cerebruma i desnog talamusa, uz korelaciju ukupnog skora DASS-21 i volumena bele mase cerebruma. U istoj grupi anksioznost je korelirala sa volumenom bele mase cerebruma.
