Abstract

Ятрогенните лезии на екстрахепаталните жлъчни канали (ЯЛ ЕХЖП) са сериозни и понякога живото-застрашаващи специфични усложнения, които са многообразни по отношение на локализация, тежест на увредата, клинични прояви, време на установяване, диагностични и лечебни подходи. В до-лапароскопската ера тази патология се е състояла основно от стенози на хепатико-холедоха. Но честотата на ЯЛ ЕХЖП нараства значително успоредно с широкото навлизане на лапароскопската холецистектомия по целия свят.Веднъж причинена, билиарната увреда изисква адекватен мултидисциплинарен подход.Важността на проблема е провокирала изследователския интерес и множество проучвания анализират анатомичните, клиничните, диагностичните и лечебните аспекти на ЯЛЕХЖП. Правени са редица опити за създаване на универсална класификация, но всяка от тях има известни ограничения.Предлаганият литературен обзор представя повечето от по-известните класификации с коментари на предимствата и недостатъците им.

Iatrogenic bile duct lesions (IBDL) are severe and sometimes life-threatening specific complications of different type, concerning location, severity, clinical presentation, time of detection, diagnostic and therapeutic approach.Common bile duct/common hepatic duct stenoses represented the great majority of this entity in the pre-laparoscopic era. However, the incidence rate of IBDL has significantly increased after the rapid spreading of laparoscopic cholecystectomy worldwide. Once a lesion has been caused, an adequate multidisciplinary approach is required. The importance of the problem provoked many studies analyzing the anatomical, clinical, diagnostic, and therapeutic aspects of IBDL. Many attempts have been made to create a universal classification system, but they all have certainlimitations.This literature review represents most of theclassifications of IBDL with some comments on their advantages and disadvantages.
