Abstract

Introducción: La técnica EXIT consiste en la exposición controlada y parcial del feto para asegurar la vía aérea fetal. Requiere un nivel óptimo de relajación uterina para mantener la perfusión placentaria y prevenir su desprendimiento [1]. Las patologías fetales reportadas con mayor frecuencia son los teratomas cervicales y la obstrucción congénita alta de la vía aérea (CHAOS) [2]. La realización de un caso EXIT presenta un desafío para el anestesiólogo, donde es primordial la administración de una anestesia materna óptima y segura, con especial énfasis en la relajación uterina [3]. Descripción del caso: Paciente de 35 años, secundigesta, cursando embarazo de 35,2 semanas. Se interna en trabajo de parto en plan de cesárea de urgencia por diagnóstico prenatal de teratoma cervical. Se administró profilaxis antibiótica y de aspiración gástrica. Se realizó anestesia combinada epidural raquídea (CSE) con Bupivacaína hiperbárica 0,5% 7,5 mg intratecal. Luego, se procedió a realizar anestesia general: se administró Propofol 200 mg, Rocuronio 70 mg, Fentanilo 50 mcg. Se procedió a intubación orotraqueal con Videolaringoscopio C-MAC. Monitoreo mínimamente invasivo con FloTrac - EV1000. Se inició infusión endovenosa de Remifentanilo a 0,2 mcg/kg/min y Nitroglicerina (NTG) a 0,1 mcg/kg/min. Para el mantenimiento de la hipnosis, se utilizó Sevoflurano, que no superó el 2 %. Se administró soporte vasoactivo con Fenilefrina en infusión continua. Finalizada la cirugía, se revierte relajación neuromuscular con Sugammadex 400 mg. Discusión: La relajación uterina para EXIT es comúnmente lograda mediante la administración de gases halogenados. En este caso, se procedió a utilizar una técnica multimodal de relajación uterina con el objetivo de disminuir la administración de Sevoflurane y sus posibles consecuencias hemodinámicas e hipnóticas a nivel materno; como así también disminuir la exposición fetal. Dicha estrategia consistió en la utilización de Nitroglicerina en combinación con concentraciones menores de Sevoflurane. Esta técnica anestésica no inmoviliza al feto por lo que no provee las condiciones óptimas para la realización de la cirugía fetal en forma segura. Por lo tanto, es necesaria la administración fetal intramuscular de relajante neuromuscular y Fentanilo. El monitoreo hemodinámico mínimamente invasivo materno debería ser considerado para la realización de procedimientos de EXIT, donde se combina el impacto de la vasodilatación producto de la anestesia subaracnoidea o combinada (CSE), la administración de Sevolfurane y de Nitroglicerina.
